The Surgery Pod - located in the waiting room - allows you to do basic checks such as BP and weight. Please ask the reception staff to show you how.

Online Bookings

You can now make and cancel appointments online. For more information, select the 'Appointments' then select 'Booking Appointments Online'.

If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

registration_formWe will register all those living within the practice boundary area. Patients from outside this area may be registered at the discretion of the Duty Doctor. Photo Identification is required – passport or driving licence. Also 2 documents with your address eg. Council Tax bill or bank statement.

Registration depends upon full completion of a registration form and New Person Health Check (NPHC).Your registration will then be processed within 3 working days. NPHC is available and recommended to all new patients – please make an appointment with the Health Care Assistant

For children under five you must provide a record of immunisations – a Red Book or child immunization update form is necessary.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Patients living outside of the practice boundary area, may be reigistered at the discretion of the duty doctor. Please speak to reception upon arrival.
